1. Download an application (below).
2. Submit the application, copies of pertinent records (prior evaluations, teacher reports,
current IEP), and a recent piece of writing your child has done.
3. Upon receipt of your documents, we will contact you to schedule a one-hour consultation
with our Director of Clinical Services. The purpose of this meeting is to gain a more complete understanding of your
concerns and your child's needs as well as to determine what services are necessary to meet those needs. It also
guides us with selecting the clinician on our staff who will best meet your child's needs. This is a time for us to
get to know you and your child and for you to learn more about how we can help him/her be more successful.
High school students must attend this meeting. Younger students are not required to attend,
though we may ask to meet them on occasion.
